Food Establishment Inspection Report - From 10/11/18 thru 10/17/18

The Food Establishment Inspection Report is a weekly report that appears Sundays. Information is taken from reports provided by the Bi-City-County Health District, and individual reports can be viewed at its web-site, http://amarillo.gov/departments/community-services/environmental-health/food-inspections. The district is now using a numerical grading system, with a score of 100 points being equivalent to zero demerits.

100 points – No major violation at the time of inspection

85 points – Considered an average score

70 points or below – Requires re-inspection within 24 hours

COS – Corrected on site during inspection

These establishments received the following scores for being out of compliance as stated:

(C/79) Carniceria La Popular, 1505 S.E. Third Ave. Food must be protected during storage, preparation and display; lids must be on food containers while in coolers; employees must wear gloves when preparing ready-to-eat items; hands must be washed before putting on gloves; eggs stored over ready-to-eat food in walk-in cooler; sanitizer must be used to clean food contact surfaces; tongs used for pastries must be cleaned every four hours; dirty knives found with clean knives in meat market; ice machine needs to be cleaned. COS. Employee items must be stored away from items sold to public; employee drinks must have a lid and straw; wiping rags must be stored clean and dry or in a sanitizer solution; need to clean wall in meat market, wall in bakery with windows and meat coolers; potentially hazardous food must be date marked; date marking system needed and must be used properly; bulk items not in original container must be labeled with common name (ex. sugar, flour, salt); pastry items stored in enclosed glass doors need to be labeled with common name and a list of ingredients if made with more than two. To be corrected within 10 days. Shelves in walk-in cooler are rusted and need to be replaced or resurfaced; gaskets on prep cooler need to be replaced (repeat violation). To be corrected within 30 days. Adequate hand washing facilities needed in bakery/tortilla area; hand sink in back prep area not easily accessible (repeat violation). To be corrected within 60 days. Ceiling in food preparation areas must be smooth, easily cleanable and non-absorbent. To be corrected within 180 days.

(B/84) Chop Chop Rice Co., 2818 Wolflin Ave. Chopapillas in cold holding at improper temperature; veggies stored on counter; all coolers must have a thermometer in a clearly visible area; hand sink blocked; scoops stored in refrigerated products; scoop handles must not touch product when stored in shelf stable products; ice buckets must be stored inverted; can opener and holster stored dirty; bulk product not in original containers must be labeled. COS. Need to clean floor drain and area surrounding dish pit, floor under back soda machine and walk-in floor and kickboard; covered trash can needed in women’s restroom; front reach-in cooler not maintaining proper temperature. To be corrected within 10 days. Potentially hazardous items held more than 24 hours must be date marked and discarded after seven days. To be corrected within 90 days.

(A/94) Denny’s #8557, 9601 E. I-40. All items in freezer need to be covered. COS. Ware washing area needs to be cleaned more often and food build-up discarded; back door needs self closer; condensate leak in front reach-in; ice build-up in serving line freezer; need to clean gaskets in serving line reach-ins and shelves and doors in serving line; tile around drains in wash room need to be repaired and grout replaced. To be corrected within 180 days.

(A/91) Domino’s, 301 N. 23rd St., Canyon. Pizza loaded into bag sitting on floor (bag used to hold food cannot make contact with floor as contamination of food could occur); all spice containers have food build-up and need to be cleaned and sanitized. To be corrected within three days. Hand sink blocked. To be corrected within 10 days. Employee jacket sitting on top of food; employee phone in kitchen; all employees must have hair restraints and beard guards. To be corrected within 90 days.

(A/97) Donut Stop, 515 E. Amarillo Blvd. All bulk items not in original containers must be properly labeled; containers being used for multiple items must have old label removed and a new label added for intended item; damaged trays used for donuts no longer smooth, durable or easily cleanable and need to be replaced; area above ware washing unit needs to be cleaned and sanitized. To be corrected within 180 days.

(A/96) Executive Inn & Suites, 2100 S. Coulter St. Employee food and beverage items must be stored separately and labeled to easily distinguish from public food service products. COS. Ceilings in buffet area and kitchen must be smooth, durable, non-absorbent and easily cleanable; hand sink and employee beverage cooler need to be labeled. To be corrected within 180 days.

(C/73) Extend A Suites, 3411 W. I-40. Food out of date; chemicals stored directly adjacent or above food product; warmer must be turned up and temperatures monitored by staff. COS. Thermometer needed in chest freezer; floors, counters and tables must be cleaned more frequently and thoroughly; hot food must be held above 135F; back flow device (vacuum breaker, anti-siphonage) must be used on spigot with a hose that is used to fill mop buckets; certified food manager needed and certificate must be posted in view of public; products and single serve items must be stored six inches off floor at all times; food product may not be stored unshielded under plumbing fixtures; a sign is needed directing consumers not to use soiled wares and utensils on return trips to buffet line; items on buffet must be labeled; employees must monitor self-serve areas to prevent contamination by guest; single service utensils must be inverted so that consumers do not contaminate food contact surfaces when using/fetching; bulk milk dispensing tube must be cut at an angle with no more than one inch protruding from chilled head; potentially hazardous products opened or prepared in house must be labeled with a date if held more than 24 hours; date marked items must be discarded after seven days; hand sink blocked; hand sink must be labeled and stocked with soap and sanitary towels at all times. To be corrected within 10 days. Mop sink area must be smooth, durable, non-absorbent and easily cleanable; seal hole in wall, paint walls and repair any water damage. To be corrected within 180 days.

(A/92) Forest Hill Elementary, 3515 E. Amarillo Blvd. Build-up in ice machine (ice machine must be cleaned and sanitized on a regular basis); food debris in hand washing stations (stations must be used for hand washing only). COS. Water line in mop room connected to water heater is leaking. To be corrected within three days. Hot holding units with torn gaskets; cutting boards heavily scarred and no longer easily cleanable and must be resurfaced or replaced. To be corrected within 10 days. Ventilation system has dust and debris build-up; intake and exhaust air ducts must be cleaned and filters changed so they are not a source of contamination by dust, dirt and other materials. To be corrected within 180 days.

(A/99) Groovy Pop Gourmet Popcorn, 3363 Bell St. Partition between hand sink and three-compartment sink is broken and no longer easily cleanable. (Replace with material that will be more durable). To be corrected within 180 days.

(F69) IHOP, 2100 S. Western St. Tomatoes in grill cold bar and server cold bar at improper temperatures; all coolers must have a thermometer; a probe thermometer must be available to cooks to ensure safe cooking temperatures; sanitizer must be tested each time it is diluted; dishwasher must ensure equipment is functioning properly; ready-to-eat product must be separated from raw food items during storage, prep and display; lemons in adulterated condition; tomatoes not stored properly in walk-in cooler. COS. Grease storage bin must be cleaned; refuse areas must be maintained so they do not attract pests or contaminate surrounding area; grease and refuse storage containers must be closed when not in use; clean wiping cloths must be stored to prevent contamination before use; wet wiping cloths must be stored in sanitizer solution when not in use; ventilation and lighting above cook station and server station must be cleaned and dusted; electrical/wiring is exposed on server station, not easily cleanable; any melted utensils or ones no longer easily cleanable must be removed from use; floors in all areas of kitchen are slick and not effectively cleaned, considerable build-up under cook station; floors and walls must be cleaned thoroughly; mechanical dishwasher not dosing sanitizer due to break in line; ice scoop may not be stored in ice when machine is self regenerating; coffee pots must be washed, rinsed and sanitized in between customers; bulk milk dispenser with build-up, spilt milk; bulk dispenser tube must be cut at an angle no more than one inch protruding from chilled head; single service items must be protected from contamination during storage and use (repeat violation); various items out of temp in cold hold on server station and on cook line; water damage to ceiling and walls in dish pit; storage areas need to be cleaned and several areas need to be organized; any unused items must be removed; various items stored without covers; build-up on walk-way from taking out trash/grease removal; personal drinks must be stored in an area that will not contaminate product or food contact surfaces. To be corrected within three days. Critical violations indicate active managerial control is lacking; monitoring staff behavior, ensuring proper training and safe operating procedures are being followed must be effectively ensured by management. To be corrected within 30 days. This is a failed inspection.

(B/80) La Pasadita Burritos, 2730 N.E. 16th Ave. Meat on counter at improper temperature; several items in cold hold improperly cooling; employees did not wash hands before putting on gloves; employee prepared ready-to-eat food with bare hands. COS. Food handler certifications must be obtained by all employees within 60 days of hire; several items without date marking and proper labels; knife with worn blade and handle; containers used to store salsa must be smooth, durable and easily cleanable; current mayonnaise containers may not be repurposed for salsa; several items without covers; Ansul system must be inspected every six months; all unnecessary items stored outside establishment must be removed; cardboard boxes being used to cover ventilation returns must be removed; screens or grates must be installed on exposed ventilation returns; Due to a number of priority violations, lack of knowledge and food safety is shown. To be corrected within 60 days.

(B/82) Palo Duro High School, 1400 N. Grant St. Hand washing stations blocked and not properly stocked with soap and paper towels; items in freezer must be covered and protected at all times; condensation and ice build-up found on ice paddles and food items in freezer; chemical bottles must be properly labeled; chicken patties and meatballs in hot holding at improper temperatures; single-use utensils must be stored properly with all handles facing same direction, dispensed individually or wrapped individually to prevent contamination; scoops for bulk products must be stored with handle out of product; ice machine must be cleaned and sanitized; sanitizing solution too weak. COS. Milk cooler lock hole needs to be repaired with easily cleanable and non-absorbent material; freezer must be repaired to prevent condensation build-up; condensate build-up in freezer and cooler must be removed. To be corrected within 10 days. Coving around kitchen needs to be repaired in multiple areas; all areas of kitchen floor must be repaired and made durable, non-absorbent and easily cleanable; back exit door needs new weather strip or seal to prevent insect or other contaminants from entering kitchen. To be corrected within 180 days.

(A/96) Red Robin, 8720 W. I-40. Wares stacked wet; ensure all containers that hold various items remain clean; containers that hold flatware or other products must be cleaned at least every 24 hours. COS. Food manager certificate must be registered with City of Amarillo. To be corrected within 10 days.

(B/84) Shi Lee’s Barbecue & Soul Food, 1213 S.W. Third Ave. Microwaves need to be cleaned. To be corrected within two hours. Unused equipment and pans in storage area need to be removed or cleaned and protected while not in use. To be corrected within 10 days. An indirect connection needed on three-compartment sink; no certified food manager; food handler cards needed; utensils must be inverted to prevent hand to mouth contact; all bulk items not easily identifiable must be labeled; storage area and shelves need to be cleaned and free of dust; Ansil system needs to be inspected and cleaned. To be corrected within 30 days. Outdoor storage room doors need to have proper seals and no light shining through. To be corrected within six months.

(A/95) Southwest Montessori Christian Academy, 4100 Republic Ave. Hand sink needs to be installed in food prep area; existing food prep sink needs an indirect connection/air gap to prevent backflow; carpet in food prep sink and hand sink areas must be removed; underside of cabinets above food prep area needs to be painted to be easily cleanable; mop sink needs back flow preventer installed. To be corrected within 180 days.

(B/85) The Spotted Pony, 2042 Paramount Blvd. Soda dispenser holster must be cleaned and sanitized; ice bucket must be stored inverted. COS. Food manager must be registered with City of Amarillo (repeat violation; re-inspection fee charged); chemical testing strips needed for chlorine and quaternary ammonia; thermometer needed in beer cooler. To be corrected within 10 days. Three-compartment sink must have indirect connection. To be corrected within 90 days. Sealant/caulk in walk-in cooler no longer easily cleanable and must be resealed; areas below three-compartment sink in back kitchen and blowers in walk-in need to be cleaned; storage areas need to be cleaned and organized; any equipment or other items not being used need to be removed from establishment; floors not easily cleanable (repeat violation); bathroom doors must self close, self latch from fully open position. To be corrected within 180 days.

(A/95) Starbucks Coffee Co., #14191, 3512 E. I-40. Hand sink blocked. COS. Food manager certificate needs to be registered with City of Amarillo (repeat violation); hand washing sign needed on front hand sink. To be corrected within 180 days.

(A/97) Starbucks Coffee Co. #6757, 3240 S. Soncy Road. Blenders need to be cleaned every four hours along with all other utensils, especially those containing dairy. To be corrected within two hours.

(B/82) Sticky Cheese Chips Deli, 3701 Plains Blvd. Product may not be stored/held past date mark; raw meat and eggs may not be stored above ready-to-eat food; food manager certificate must be registered with City of Amarillo on yearly basis. COS. Ready-to-eat products and commercially processed food must be date marked and discarded after seven days; three-compartment sink used for hand washing; replace as necessary any plastic bowls stained or warped from heat; hair restraints must be used; build-up on fan in kitchen must be cleaned; back door sweep needed; front door not sealing properly. To be corrected within 180 days.

(A/97) Toot ‘n Totum #12, 3365 Bell St. Ceiling panels in kitchen area need to be placed back into position; less well traveled areas under soda machine and behind reach-in coolers need to be swept and mopped more often; clean wall by floor drain; equipment used to prepare food for public must be commercial grade; microwave needs to be sufficient to maintain durability for type of use; plate broken; area near nozzles for chillers/slushy drink must be cleaned more effectively. To be corrected within 180 days.

(A/96) Toot ‘n Totum #81, 2601 Paramount Blvd. Raw product may not be stored above ready-to-eat product; chemicals may not be stored on food contact surfaces near single service items. COS.

(A/96) World Market, 3350 S. Soncy Road. Single-use stir straws must be individually wrapped when available for public use. COS. Sanitizing solution not available to properly wash, rinse and sanitize coffee machines and nozzles. To be corrected within three days. Coffee machines being used to distribute samples for public consumption were removed.
